On [Date of first meeting], we met to discuss your unsatisfactory performance pertaining to your failure of meeting production deadlines on a consistent basis.

I suggest that you schedule weekly production meetings with your staff and myself so that we may identify and review the complications that are evident in the production, and work toward a solution to these hang ups. Also, I encourage you to prioritize your work schedule so that all rush projects are completed in a timely manner.

I will assist you in any way I can, to remedy the problem. If the problem lies in insufficient personnel resources, I will make a recommendation to increase the number of your staff; however, if the problem lies elsewhere and these matters are not rectified, the company will have no alternative but to undertake further disciplinary or corrective action, including and not limited to a suspension and/or another evaluation/probation period.
